TRT vs hCG: replacement or stimulation?
TRT and hCG both raise testosterone — but they work in opposite directions. TRT replaces testosterone from outside. hCG tells your own testes to make more. The choice often comes down to fertility and the cause of your low T.
How each one works
- TRT — direct replacement. Testosterone is delivered via injection, gel, cream or patch. Your pituitary detects the high level and stops sending LH, so your testes shrink and stop producing testosterone (and sperm).
- hCG — stimulation. hCG looks like LH to your testes and tells them to keep making testosterone. Your own production stays active. Sperm production is preserved.
When TRT is the answer
- Confirmed primary hypogonadism (testes can't respond — high LH already).
- Family complete, fertility not a concern.
- Older men where stimulation alone isn't enough.
When hCG is the answer
- Younger men with secondary hypogonadism who want to preserve fertility.
- Men coming off long-term anabolic steroid use trying to restart natural production.
- Men on TRT who want to maintain testicular size and some sperm production (combined therapy).
Combination therapy
Some endocrinologists prescribe low-dose hCG alongside TRT to keep the testes active during treatment. This adds cost and complexity but maintains testicular size and partial fertility. Discuss this option specifically if fertility matters to you.

| TRT | hCG | |
|---|---|---|
| Effect on natural T | Suppresses | Stimulates |
| Fertility | Reduces | Preserves |
| Testicular size | Shrinks | Maintained |
| Frequency | Weekly–quarterly depending on form | 2–3 injections per week |
| PBS coverage | Yes (testosterone) | Specific indications |

What is hCG?+
Human Chorionic Gonadotropin (hCG) is a hormone that mimics LH and stimulates the testes to produce their own testosterone. Unlike TRT, it preserves testicular function and fertility.
Can I take hCG instead of TRT?+
Sometimes — particularly in younger men with secondary hypogonadism who want to preserve fertility. In older men or those with primary hypogonadism (testicular failure), hCG alone usually isn't enough and TRT or a combination is needed.
Is hCG covered by the PBS?+
Subsidy eligibility depends on the indication and specialist assessment. Treatment options and costs are discussed with your doctor.
