Brain fog and low testosterone: causes, tests and treatment

Poor concentration, slow word recall, and mental fatigue are commonly under-recognised symptoms of low testosterone in Australian men over 30.

    Why low testosterone affects cognition

Testosterone receptors are densely expressed in the hippocampus and prefrontal cortex — the brain regions handling memory and executive function. Hypogonadism is also associated with sleep fragmentation and low mood, which compound cognitive symptoms.

    Other causes to rule out

    • Hypothyroidism — measured by TSH.
    • B12 or iron deficiency — common and reversible.
    • Obstructive sleep apnoea — particularly if you snore or wake unrefreshed.
    • Depression and anxiety — overlap heavily with cognitive symptoms.
    • Long COVID — now a recognised differential.

    The Australian work-up

    1. Take the free ADAM symptom screen and bring it to your GP.
    2. Morning fasting blood test: total testosterone, SHBG, LH, FSH, FBC, iron studies, B12, TSH, HbA1c.
    3. If testosterone is low, a confirmatory test on a different morning.
    4. If confirmed, referral to an endocrinologist for PBS-subsidised TRT.

    People also ask

Can low testosterone cause brain fog?+

Yes. Men with confirmed low testosterone frequently report poor concentration, slow word recall, and mental fatigue. Because brain fog has many possible causes, a doctor needs to assess it.

    How is brain fog tested?+

There is no direct test for brain fog. A morning hormone panel (testosterone, SHBG, LH, FSH), thyroid function, vitamin B12, vitamin D, iron studies, and HbA1c rule in or out the common reversible causes.

Can treatment improve focus and memory?+

Research on testosterone and thinking is mixed. Brain fog has many causes, including poor sleep, stress, depression and other medical conditions, so a doctor will look at all of these. Any treatment is a doctor's decision, and results are not guaranteed.
